Is it necessary to get health checkup done ? What if I have to take medicines everyday?

up1

Dr Paula Goel , Fayth Clinic

Does the thought of taking medicine create irrational fear in your heart ?

If so, you may suffering  from a rare phobia called pharmacophobia which is  the irrational fear of taking medication. Patients who suffer from pharmacophobia experience palpitations, panic attacks, racing heart rates, trembling, nausea, and even weakness. These are all traditional anxiety symptoms that stem from an overactive “fight or flight” response in the body.

When these individuals have to go for medical assessment or even for routine annual health screening , they get very stressed out . The first thought which enters their head is “ What if something is wrong and I do need to take medicines”. This creates an anxiety situation leading to fight or flight response . Sometimes , so high is the fear , that they stop taking their medications leading to aggravation of the disease process and causing a great deal of harm to themselves.

So , is it possible to overcome this fear of pharmacophobia ? Here’s the good news: it is possible to overcome your pharmacophobia. Start with these strategies to build up your anxiety coping mechanism.

Find out how the pharmacophobia started

Women sitting together in a circle in a support group.

Generally this develops after a particular negative experience in the past like side effects or aallergic effects which could have been life threatening or they could have seen a loved one suffering a serious illness which required taking very strong medications and associated with death. Even though the death may not have been as a result of the medications, the brain then draws the correlations between the two. This may cause patients to fear that they will have the same experience. Therapists often refer to this as “fearing the fear” itself.

If you can trace your fear back to when it may have started, you may also identify what caused it in the first place. Think back to negative experiences you or someone you know has had with medication. How did those experiences affect you? Remind yourself that someone else having a negative experience does not necessarily mean you will  too.

Try taking your first dose in front of your doctor

up2

If there is tremendous fear of taking medications by yourself , you could take the first dose in front of your doctor , so that you can be monitored and incase of any adverse effects , you can be treated immediately . Once you realize that there are no problems , you can overcome your phobia and start taking them at home in front of your family members .  Research tells is that people with pharmacophobia experience the most fear with the first dose. As they take their medication and witness the lack of side effects, the anxiety lessens – hopefully it lessens for you, too

Try Talk Therapy

Women sitting together in a circle in a support group.

If you still require help to deal with pharmacophobia , you could go for Talk therapy, including both Cognitive Behavioral Therapy (CBT) and Dialectical Behavioral Therapy (DBT), can both significantly help people who suffer from pharmacophobia. The first attempts to change how you perceive your fear, and thus, its outcome. The second approach teaches you to be mindful and stay in the moment. In phobias where regressions and triggers can potentially cause more harm than good if mishandled, hence finding the right therapist is vitally important

You could also try  DBTSelfHelp.com. This website contains a database of DBT-based exercises and information you can put to use right at home. Practical skills learned include distress tolerance, emotional regulation, and mindfulness

Try Exposure Therapy

up4

Exposure therapy may help to desensitize you. This form of therapy exposes you to your fear with a therapist by your side to help you work through the negative emotions you experience. By research and statistics, it shows the highest level of efficacy in resolving extreme phobias. Exposure therapy is often gradual, so a complete fix may not happen for months or even years. However, it is still a valuable long-term tool. Patients with phobias who undergo exposure therapy often come to resolve their fears completely and permanently

Change Your Dosing Method

The phobia itself may be related to a specific form of dosing rather than the medication itself. Patients may struggle with swallowing medicines , but do fine when given intramuscular injections. Others may be fine with pills, but struggle with topicals. Even the fear of choking on medication can be enough to dissuade some patients from sticking with treatment. Changing the approach to accommodate may be easier than eliminating it.

Compounding is especially useful for children with pharmacophobia, who may be more willing to take a medication that has better taste over a formula that’s bitter or sour. Pharmacists can add in flavoring to make the medication more palatable.

What’s most important is that you know pharmacophobia is nothing to be scared of . There are  plenty of patients who struggle with it, and most eventually overcome their fears and can get treated for their medical problems.  It is always best to diagnose & identify medical problems early in the course of the disease . You can, too!

Do I need to take lifelong BP medicine

Do  I  need to take lifelong BP medicine

Dr. Saurab  Goel , Interventional Cardiologist ( Wockhardt Hospital , Global hospital, Breach Candy hospital ) up1

Has your blood pressure been creeping up inspite of taking your blood pressure medications ? If so, you may be at decision time: Do you need medicine life long  or can you get those numbers down with healthier habits?

It depends on how high is your blood pressure and how is your general health and lifestyle . And what is your age.

The first step is to look at blood pressure figures . Blood pressure is always shown with a top and bottom number  like 120/80

The top number tells you the pressure when your heart beats. The bottom number lets you know the pressure between beats.

Normal blood pressure is about 120 / 80. If one or both of those numbers is higher, there’s too much pressure in your arteries. Over time, the extra pressure can cause damage — and raise your chances of heart disease, stroke, and kidney disease.

So where do your numbers fall ?

130/80 – 139/89 (stage 1 hypertension): You might need medication.

up2

These numbers qualify as high blood pressure and you need to take action. But you try out lifestyle changes first before adding drugs , unless you have other health problems.

One thing to keep in mind: Guidelines are different for older people. If you’re 60 or older, the American College of Physicians and the American Academy of Family Physicians recommend you start treatment if your top blood pressure number is 130 or higher.

140/90 or higher (stage 2 hypertension): You need medication.

At this level, your doctor is likely to prescribe medicine now to get your blood pressure under control. At the same time, you’ll also need to make lifestyle changes.

If you ever have blood pressure that’s 180/120 or above, it’s an emergency. You need medical help right away to get it under control.

While your numbers are important, they don’t tell the whole story. Other things affect the decision to take meds. Some issues to think about:

Do you definitely have high blood pressure? One high reading isn’t enough to know. Your doctor will want to check your blood pressure a few more times , over weeks or months just to make sure. Or alternatively , you could get an ambulatory BP monitoring done to identify the spikes in blood pressure

Have you checked your blood pressure at home, or only in the doctor’s office? If you don’t already have one, it’s a good idea to get a home blood pressure monitor. Lots of people have high blood pressure in the doctor’s office because they’re nervous( white coat effect ) . If your home readings are normal, your doctor may decide you don’t really need treatment. Although you may still get an ambulatory BP monitoring done to identify the spikes in blood pressure

Have you already tried lifestyle changes  and your blood pressure is still high? If your blood pressure is still too high after 3 months of healthier habits, your doctor is likely to suggest medicine. Incase you are already on medications and have followed lifestyle changes , however your Blood pressure is still high , then you need to undergo certain investigations to  rule out other causes of high blood pressure

If you have other medical problems , like high cholesterol, diabetes, or heart disease ,  high blood pressure raises your chances of getting a heart attack or stroke.

There are certain other criteria like race , age , gender , family history , habits like smoking , alcohol , being overweight or obese could also predispose to heart attack or stroke .

Some people choose to take medicine because they’re concerned about the risk of heart attacks and stroke. Others don’t like the idea of going on medication or worry about the side effects. Talk to your doctor about the pros and cons, and be open about how you feel.

Remember, high blood pressure has no symptoms. So the only way to know if it’s high or getting higher  is to check it regularly.

Medications are not an excuse to give up on healthy lifestyle . You’ll get the most benefit from your blood pressure drugs if you combine regular exercise, a good diet, and a healthy weight.

Could you have diabetes ? 5 hidden symptoms of diabetes

Could you have diabetes ? 5 hidden symptoms of diabetes

Dr Paula Goel , Fayth Clinic

up2

Did you think the only sign of being diabetic is overweight? Think again…

Around 77 million people in the India have diabetes ( 2nd largest population having diabetes in the world ) , and 90% of the diabetics are not even aware of their condition . And while diabetes is a lifelong condition & can be successfully managed once it’s diagnosed.  Delaying that diagnosis puts people at risk of serious complications, including amputation and blindness.

The  key concern for Type 2 diabetes is weight  gain ,which accounts for around 90% of the cases. Type 2 occurs when the body can no longer make enough insulin (a hormone produced by the pancreas which enables us to use sugar/glucose), or the insulin being produced isn’t doing its job properly.

Type 1, on the other hand, has absolutely nothing to do with weight or lifestyle and tends to develop during childhood when a fault in the body causes insulin-producing cells to be destroyed.

The symptoms of Type 1 and Type 2 are very similar, however in Type 1  diabetics , they occur  quickly as compared to type 2 and can land you in the hospital very rapidly , if not diagnosed in time  Type 2 is a bit different as many people put the symptoms of Type 2 with advancing age .The condition can sometimes go undiagnosed for up to 10 years, by which time complications could have started to develop.

Aside from weight , increased thirst , what are some of the other symptoms

  1. Blurred vision

964487804

If you are struggling to see the finer details or have floaters in your vision field , then it could be due to diabetes . Blood sugar disruptions may cause the eye’s lens to become dry or swollen. One or both eyes may be affected. Always get eye symptoms checked out. If its not diabetes , could be due to some other cause.

  1. Itching near genitals

Yeast infection or fungal infection and cause itching and if this occurs repeatedly , even after treatment, its worthwhile to check out for diabetes  .This is because the associated high glucose levels in blood or urine provide the perfect breeding ground for troublesome yeast.

  1. Slow healing wounds

If minor cuts, burns and grazes suddenly seem to take ages to heal, it could be a sign that something’s wrong, like a nutritional deficiency – or potentially diabetes. If not managed carefully, diabetes can lead to nerve damage and poor circulation, resulting in slower healing.

  1. Nausea and vomiting

up6

Nausea & vomiting can occur for many reasons, however, if it occurs with  increasing frequency  without any apparent cause , could be a sign of Type 1 or 2 diabetes.  Blood sugar fluctuations can disrupt the metabolism or cause dizziness (another possible symptom!), which sometimes makes people feel sick.

  1. Dizziness

up7

As mentioned above, dizziness can be a sign of diabetes, due to both low blood sugar or dehydration (brought on because diabetes can make people urinate far more than usual). If dizziness continues for more than a day or two, or the dizzy spells are lasting a long time, get checked as there could be a number of causes that need investigating. A person with diabetes can develop hypoglycemia from a decrease of food intake, or from taking too much medication (insulin or oral tablets), which results in low blood sugar levels. In this situation the person experiences dizziness or lightheadedness because the brain lacks glucose to function properly.Because diabetes is such a diverse disease with many complications, it can cause dizziness in many ways.

Yes , it is essential to check out if you have diabetes and get treated at the right time . Correct diagnosis t the right time would go a long way to prevent multiple complications .

5 Reasons Why You Avoid Health Checkup

5 reasons why you avoid  health checkup

Dr Paula Goel , Fayth Clinic 5

Over 1/3 of patients avoid health screenings or even avoid going to medical checkup due to belief systems related to health care . These belief systems could be related to previous unfavourable situations or evaluations perceived by them , their friends or relatives . This could be related to factors related to doctors , healthcare organizations and affective concerns.

1

Here are 5 reasons why people avoid medical checkups :

1.There may be a low perceived need to seek medical care as individual feels that their illness or symptom may improve with time or improve on their own  . Hence , they are willing to wait and endure the illness. Many time , such patient seek healthcare services at the time when its too late . Many do not believe in going to the doctor because they did not have any health problems

2. Many people feel it is a waste of money to get their medical examinations and tests done and when they do reach the healthcare sector , they may find the cost high as they have ignored their health problems earlier. This reinforces their belief system that healthcare is expensive . And this is very true especially if there is no health insurance to cover the medical  admission costs .

3. Time constraints play an important role especially if they have to take the day off and undergo medical tests . Many are very busy to seek medical care for their own health because clinic hours are inconvenient .

up3

4. People may avoid going to the doctor if they feel that doctor is too far or transportation is difficult or they were too sick to travel

5.  Concerns about quality of medical care and interpersonal concerns of the doctor also plays an important role. Communication with doctors may pose a problem to the patient , especially if the doctor has a heavy OPD and patient perceives that doctor has devoted less time . Many times patients feel that doctors do not take patient concerns seriously or they have general mistrust  & lack of confidence in healthcare providers . Many patients have fear of needles , which makes it difficult for them to undergo blood tests , vaccinations or surgeries .Many patients are not comfortable disclosing their health concerns and habits  ( smoking , alcohol intake etc ) to the doctors . Some even have dislike for medical recommendations ( especially on weight loss , alcohol consumption , smoking and medications for high blood pressure ) and perception that recommendations would not be useful .

Do I have to take lifelong medicine in Diabetes

Dr . Paula Goel , Fayth Clinic

2

Diabetes is a lifelong condition in which the Persons blood sugar level increases much higher then normal

There are 2 types of DM

-Type 1 : where bodys immune system attacks and destroys the cells that produce insulin

Type 2 : where the body doesnot produce enough insulin or the bodys cells do not react to insulin

Type 2 DM is more common than type 1 and more than 90 % of adults suffer from type 2

Most people need medicine to control their type 2 diabetes.

Medicine helps keep your blood sugar level as normal as possible to prevent health problems. So ,do you have to take lifelong medications in diabetes .

The answer to that remains on how well controlled your blood sugar is !

Staying healthy is very important by maintaining a healthy diet ,by keeping active and taking your medications on time .

It will also help you to control your weight and generally feel better

For some individuals , Diabetes usually gets worse over time, so your medicine or dose may need to change

And you may have to take it for the rest of your life.

However for those whose blood sugar is borderline high on medication , they can be put on low dose diabetic medications with diet and daily exercises.

You can eat a wide range of foods including fruits , vegetables , high fibre food

Keep sugar , fat and salt to a minimum

Breakfast , lunch & dinner is to be taken daily and meals should not be skipped . If diet needs to be changed , small changes may be made weekly

4

Getting the right medicine for you is important , Diabetes medicines help lower the amount of sugar in your blood. You’ll usually be offered a medicine called metformin first.

If your blood sugar levels have not lowered  after taking metformin, you may need another medicine.

Over time, you may need a combination of medicines , if diabetes in not under control. Your doctor  will recommend the medicines most suitable for you.

Insulin is not often used for type 2 diabetes in the early years. It’s usually needed when other medicines no longer work. Your doctor will explain how to take your medicine and how to store it.

5

If you need to inject insulin, they’ll show you how.

Your diabetes medicine may cause side effects.

These can include:

  • bloating and diarrhoea
  • weight loss or weight gain
  • feeling sick
  • swelling in one or more parts of your body due to a build-up of fluid under your skin

Not everyone has side effects.

If you feel unwell after taking medicine or notice any side effects, speak to your doctor.

Do not stop taking medicine by yourself without taking your doctor’s advice.

If you’re going on holiday:

  • pack extra medicine – speak to your doctor about or how much to take
  • carry your medicine in your hand luggage just in case checked-in bags go missing or get damaged
  • if you’re flying with a medicine you inject, get a letter from your doctor  that says you need it to treat diabetes

Diabetes is a lifelong disease which can be controlled . Diabetic rehabilitation which includes customized healthy diet , exercise protocols, medications and counseling go a long way

to control diabetes and decreases the requirement of medication .
